Change of Major Forms
To apply to any major in the School of Social Ecology or College of Health Sciences, students must complete the following:
- Undergraduate Petition to Change Majors.
- 120 Unit Worksheet - (if applicable) for students with 120 or more completed units wishing to declare, change or add an additional major.
- Double Major Worksheet - (if applicable) for students wishing to add a major and who have less than 120 units.
- Have completed the UC Entry Level Writing Requirement (formerly Subject A) (pre lower-division writing) requirement.

Courses that satisfy major change requirements must be completed at the time the application is submitted.
